Incident / Injury Reports
All accidents, medical problems associated with work,and incidents which might have causedinjury, should be reported on the McMaster University "Incident/Injury Report" form. Completed forms should be returned to your supervisor with a copy going to Marge Geroux, Biology Administrator in the Main office LSB218 and EOHSS within 24 hours.
